Select this option if you are a repair facility doing work for the contract holder and wish to report a claim.

Repair Facility

Please read all of the following information carefully before proceeding to the claim form:

If a failure requires emergency repair and the administrators claims department is closed, administrator will waive the requirement of pre-authorization subject to the following:

  1. Have the contract holder authorize the inspection/tear down of the unit to determine the cause of failure and cost of repair. (Inform contract holder that the cost of tear down will not be paid if USWC determines that the failure is not covered under the contract.)

  2. Determine the cause of failure, the correction required and the cost of repair.

  3. Contact administrator claims specialist at 800-233-9878 or by e-mail at CLAIMS@USWCEAGLE.COM to discuss the claim. You must obtain an authorization # and authorization amount from the administrator prior to commencing the repair.

  4. Review Administrators findings with the contract holder and obtain contract holder?s
    consent to proceed with the repair.

  5. For payment, submit the repair orders to the following address: United States Warranty Corporation, 6140 Parkland Blvd., Mayfield Heights, OH 44124 or fax to 866.876.1594.

Note: All repair orders must be signed by the contract holder and must include the USWC contract number and authorization number. All repair orders must be submitted to administrator within 30 days of authorization date.

Note: Pursuant to the terms of the service contract, USWC reserves the right to conduct an independent inspection/evaluation. You must retain all components including fluids, filters, etc... until the claim is completed.
