Mechanical Breakdown
Select this option to report a claim if you are the contract holder, your vehicle is operational and you are able to continue traveling in a safe/reasonable manner.
Emergency Breakdown
Select this option to report a claim if you are the contract holder, your vehicle is inoperable and you are unable to travel in a safe/reasonable manner.
Repair Facility
Select this option if you are a repair facility doing work for the contract holder and wish to report a claim.
Mechanical Breakdown
Please read all of the following information carefully before proceeding to the claim form:
- Protect vehicle from damage which may result from continued operation.
- Transport vehicle to dealer or the nearest licensed repair facility or call us or the administrator for a referral.
- Call administrator with a diagnosis and an estimate of the parts and labor to receive a repair authorization number before repair work is started.
- Administrator reserves the right to review maintenance records, have access to any O.E.M. vehicle analysis software reports, inspect the vehicle/parts in a failed state prior to authorizing repair, request and receive additional documentation, obtain a signed proof of loss or release and evidence of title.
- Proper documentation for reimbursement must include a receipt detailing a breakdown of parts and labor, vehicle make, model, VIN number, mileage/hours and the authorization number. Documentation must be received by administrator within thirty (30) days of the authorization date in order to be eligible for payment. Authorized claims will not be valid after 90 days from the date authorized.
