Mechanical Breakdown
Select this option to report a claim if you are the contract holder, your vehicle is operational and you are able to continue traveling in a safe/reasonable manner.
Emergency Breakdown
Select this option to report a claim if you are the contract holder, your vehicle is inoperable and you are unable to travel in a safe/reasonable manner.
Repair Facility
Select this option if you are a repair facility doing work for the contract holder and wish to report a claim.

Please read all of the following information carefully before proceeding to the claim form:
If a failure requires emergency repair and the administrators claims department is closed, administrator will waive the requirement of pre-authorization subject to the following:
- Your vehicle is inoperable in that you are unable to proceed with your travel in a safe/reasonable manner.
- Prior to commencing repair, you must call administrator and furnish the answering service with: your name, contract number, date, time, telephone number of the repair facility, the nature of the failure and state that you are proceeding with an ?Emergency Repair?. Examples of an emergency failure - failed fuel pump, starter motor, etc. Examples of a non emergency failure: air conditioning, microwave range, deluxe appliance, leveling jack, slide-outs, etc. Administrator reserves the right to evaluate the ?failure? in accordance with the other terms and conditions of this contract
